Reduced capacity at the surgery

We continue to experience reduced capacity due to sickness across our GP team and will continue to do so at least until the end of February.  All appointment requests are triaged by a GP and appointments are allocated according to clinical need. There may be times that we have to switch eConsult off on-line to help us manage demand.  We will continue to take requests over the telephone or at our front desk but we may have to restrict requests to urgent medical problems only.

How to Register

To register with the practice you must reside within the practice area. Registration will only be completed once the Practice has received your forms and emailed you to confirm.

Prior to registering with our Practice we ask patients to ensure they have a 4 week supply of medication from their current Practice. This allows time for us to receive your records ahead of initial request for medication.

All new patients will be requested to complete a lifestyle questionnaire when applying to join the practice and we like all new patients to have a health check with a member of our clinical team within one month of applying. We cannot fully register you with the practice until this has been carried out. If you have a complex health issue you may need to see a Doctor.

You can, of course, see a Doctor if you have an urgent problem in the meantime.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

We have the right to refuse to register patients if they have a violent flag against their record.

Register online

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Calling all Veterans

We are trying to identify patients who are military veterans. Did you serve for at least one day as either a Regular or Reservist in the British Armed Forces?

Organ Donation
