HOW
DO I....
OBTAIN
A REPEAT PRESCRIPTION?
If you require regular medication,
the Doctor may enter the prescription on the computer. This will enable you to
obtain monthly prescriptions by presenting your computerised repeat slip at the
Medical Centre. Please allow two complete working days before collection and
remember to make allowances for weekends and public holidays. You may collect
your prescription any time after 2.30pm on the appropriate day. Alternatively,
you can post your repeat slip enclosing a SAE. We prefer not to accept telephone
requests for repeat prescriptions unless you are housebound. Telephone requests
can sometimes lead to mistakes being made. However, if you are housebound and
do need to telephone, please do so between 10.30am and 4.30pm.
Patients on long term medication must consult the Doctor at six-monthly intervals
for a check on their medication
THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
