REGISTER WITH THE PRACTICE?
To register please call at reception
bringing your medical card, if available. To register with the practice you must
reside within the practice area. All new patients will be requested to complete
a lifestyle questionnaire when applying to join the practice and we like all new
patients to have a health check with the Nurse or Doctor within one month of applying
and we cannot fully register you with the practice until this has been carried
out. You can, of course, see a Doctor if you have an urgent problem in the meantime.

CHANGE MY DETAILS?
Please let us know if you move or
change your name or telephone number so that we can update your medical records.
If you move out of the Practice area, it will be necessary for you to register
with another Doctor. The Receptionist will help you with queries about practice
boundaries.

OBTAIN TELEPHONE ADVICE?
You may seek advice from, or leave
a message for, a Doctor if you do not need to be seen. Please ask the Receptionist
who will arrange this for you. You may be asked to telephone back at a given time.

MAKE AN APPOINTMENT?
Although you register with one of
the Doctors by name, you can, by choice, consult any of the Doctors, provided
there is an appointment available. All consultations are by appointment.
However, in order to make the best use of Doctors' and Nurses' time you may
be asked some questions when you telephone the Medical Centre to make an appointment.
This is so we can offer you an appointment with the most appropriate person to
deal with your condition and you may be offered an appointment with a Doctor,
Nurse Practitioner, Specialist Nurse or one of our Practice Nurses. Alternatively,
you may be given the option of a telephone consultation with a Doctor or Nurse
Practitioner. Urgent cases will be offered an appointment the same day.
If you require a non-urgent appointment please telephone the Medical Centre on
604084 after 2.00pm as the mornings are always very busy.
Please remember that an appointment is for one patient only. IF YOU CANNOT
KEEP YOUR APPOINTMENT, KINDLY LET US KNOW AS ANOTHER PATIENT WILL ALWAYS TAKE
ADVANTAGE OF YOUR CANCELLATION.

OBTAIN A HOME VISIT?
Requests for a home visit should
be made before 10.00am, except in cases of emergency. The Receptionist will ask
you the reason for the visit. Please give her the information she requests so
that the Doctors can give priority to urgent cases.
A request for a home visit may be refused if the Doctor considers that it would
be more appropriate for the consultation to take place at the Medical Centre.
Lack of transport is not a valid
reason for a home visit.

OBTAIN TEST RESULTS?
Smear tests, urine tests, blood tests, etc are carried out for the benefit of your health. PLEASE ENSURE YOU CONTACT THE MEDICAL CENTRE FOR THE RESULTS. If telephoning or calling in, please do so between 2.00 and 6.00pm and not during morning surgery.
Most test results are back in approximately five to seven days, though smear test results take between four to six weeks (the Nurse will advise you).
Pregnancy tests are usually available within two days. Please note pregnancy test results will not be given out over the phone.
All results will only be given to the patient and NOT to relatives, for reasons of confidentiality. The Receptionist will only be able to state that the result is normal or that you will have to see the Doctor.
Please do not expect the Receptionist to have any other information regarding the test.
